Our team is thankful that you've chosen our office to meet your dental needs. Your initial appointment with one of our providers will take an hour and that includes a comprehensive exam and digital x-rays. Prior to your initial consultation, kindly provide the following information to help us:
- Complete the digital forms we texted or emailed you.
- Forward any x-rays in a JPEG format prior to your appointment.
We require all new patient forms be completed before your arrival to your appointment. If you have dental insurance, please provide that to us when you're making your initial call to the office for your appointment.
One of our primary concerns is the security and privacy of your personal data. Our team has taken every precaution to best protect it.
RADIOGRAPHS (X-RAYS)
If your previous dental clinic has taken radiographs (x-rays), you may request them to forward a copy to our office @ smile@hillsdaledentalcare.com. Otherwise, kindly pick up a physical copy from your previous dental clinic and bring them to our office. If additional films are necessary, they can be taken at our facility and an additional charge may incur.
